Add link to Google Form to request access

To allow new users to request access, we add a link to a Google Form.

Testing Instructions

• Checkout this branch and update and server
• Go to http://localhost:4545/
  • Ensure you see the "Request access" link in the login page
  • Ensure it takes you to the right form
